چکیده

1. In eight patients being treated by chronic dialysis the relation of magnesium metabolism to the magnesium concentration of the dialysate was investigated. During four periods of 2-6 weeks the magnesium concentrations of the dialysis fluid were 0'44±0'15 mg/IOO ml, 0'70±0'24 mg/IOO ml, 1'83±0'18 mg/IOO ml and 1'97±0'21 mg/IOO ml. 2. During all periods the concentration of plasma magnesium and ultrafiltrable magnesium was above normal. The concentration of erythrocyte magnesium was below normal with the lowest concentration in the bath, while all other dialysate concentrations yielded readings above normal values. 3. During dialysis the concentration of plasma magnesium and ultrafiltrable magnesium dropped only during the periods with low concentrations of magnesium in the dialysate. During dialysis with the lowest concentration the values for erythrocyte magnesium showed a significant rise whereas with high concentrations the concentration fell significantly. The changes in erythrocyte magnesium proved to be independent of the calcium content of the dialysis fluid. 4. The maximum loss of magnesium to the bath solution amounted to 710 mg, with the magnesium content of the bath being only 0'27 mg/IOO ml. The net transfer of magnesium to the blood of the patient amounted to 102 mg with a dialysate magnesium concentration of 1·86 mg/IOO ml. 5. It is concluded that low dialysate magnesium concentrations result in erythrocyte magnesium deficiency whereas high concentrations cause magnesium overloading. A magnesium concentration of 0'7 mg/IOO ml is suggested for the bath solution used in regular dialysis treatment.

ALUMINUM TOXICITY: A REVIEW IN RELATION TO CHRONIC RENAL FAILURE PATIENTS M AINTAINED ON REGULAR HEMODIALYSIS

Aluminum is present in very small amounts in living organisms but abundant in the environment. A growing literature links with the biochemistry of aluminum and also with a series of diseases in chronic renal failure patients on treatment with hemodialysis. The initial description of potential aluminum toxicity in renal failure patients relates to description of dialysis encephalopathy syndr...

Effect of magnesium on nerve conduction velocity during regular dialysis treatment.

Serial nerve conduction velocities in the peroneal and ulnar nerves have been measured in 10 patients on regular dialysis treatment over a three year period. Each patient alternated between phases on dialysis with magnesium-containing dialysate (1·5-1·7 m-equiv/l.) and phases on `magnesium-free' dialysate (0·2 m-equiv/l.). Plasma magnesium concentrations were high both pre- and post-dialysis du...

The relationship between the magnesium concentration in the dialysis fluid used and in the plasma and erythrocytes of patients with chronic renal failure being treated by regular haemodialysis.

1. Plasma and erythrocyte magnesium concentrations have been determined in fifty-five patients on regular haemodialysis and in thirty-two healthy subjects. The dialysis fluid magnesium concentrations of the former were 0.58,0.9 and 1.73 m ~ . 2. There was a linear correlation between the plasma and erythrocyte magnesium concentrations of normal subjects and dialysed patients whose dialysis flui...

Association between serum magnesium levels with lipids profile in patients undergoing peritoneal dialysis and hemodialysis

Introduction Renal replacement therapy is a general term applied for several techniques which are used in the treatment of patients with end-stage renal failure. It includes different methods such as hemodialysis, peritoneal dialysis, and renal allograft transplantation (1,2). The efficacy of peritoneal dialysis as a method of renal replacement therapy has been proven. The purpose of dialysis i...

Magnesium in chronic kidney disease Stages 3 and 4 and in dialysis patients

The kidney has a vital role in magnesium homeostasis and, although the renal handling of magnesium is highly adaptable, this ability deteriorates when renal function declines significantly. In moderate chronic kidney disease (CKD), increases in the fractional excretion of magnesium largely compensate for the loss of glomerular filtration rate to maintain normal serum magnesium levels. However, ...
